How Does Venclexta Work?
Venetoclax inhibits BCL-2 (B-cell lymphoma 2), an anti-apoptotic protein overexpressed in many hematologic malignancies including CLL and AML. By blocking BCL-2, venetoclax restores the apoptosis signaling that cancer cells suppress to survive, triggering cell death in BCL-2-dependent tumor cells.
Dosage & Administration
CLL: 5-week ramp-up (20→50→100→200→400 mg/day) to reduce tumor lysis syndrome risk, then 400 mg daily. AML: 400 mg daily (with dose reductions for azole antifungals). Ramp-up is MANDATORY for CLL/SLL — do not skip.

General Warning
TUMOR LYSIS SYNDROME (TLS) — potentially fatal; mandatory ramp-up required, hydration, uric acid prophylaxis, and lab monitoring. Neutropenia — dose-limiting. Infections. Embryo-fetal toxicity. Significant CYP3A4 drug interactions (strong inhibitors dramatically increase venetoclax levels).

The ramp-up schedule (starting at 20 mg and increasing weekly) is mandatory for CLL to reduce the risk of tumor lysis syndrome (TLS) — a life-threatening condition where rapid tumor cell death releases dangerous amounts of electrolytes into the bloodstream.
