Absolute Contraindications
🚫 Two Biologics Simultaneously
Never combine any two biologic medications. Dual biologic therapy dramatically increases the risk of serious and life-threatening infections. This includes combining any two of: TNF inhibitors, IL-17 inhibitors, IL-23 inhibitors, IL-6 inhibitors, JAK inhibitors, anti-integrins, IL-4/IL-13 inhibitors, or T-cell modulators.
🚫 Anakinra + TNF Inhibitors
Combining anakinra (Kineret) with any TNF inhibitor is contraindicated due to significantly increased infection risk and no added benefit.
🚫 Live Vaccines on Any Biologic
Live vaccines (MMR, varicella, live flu nasal spray, yellow fever, oral typhoid) are contraindicated while on most biologic medications due to the risk of vaccine-strain infection in immunocompromised patients.
