Drug Interactions with Biologic Medications
What to avoid, what's safe, and how BiologicHealthPlus screens your medications
Key Takeaways
- Never combine two biologic medications — serious infection risk
- Live vaccines are contraindicated with most biologics
- Methotrexate is commonly and safely combined with many biologics (reduces immunogenicity)
- Always tell every provider and pharmacist about all medications you take
- BiologicHealthPlus performs a full drug interaction review at every refill
Critical Interactions: Never Combine
⚠️ Biologic + Biologic = Contraindicated
Two biologic medications should never be used simultaneously. Combining biologics dramatically increases the risk of serious and life-threatening infections. This includes combining any two of: TNF inhibitors, IL-17 inhibitors, IL-23 inhibitors, IL-6 inhibitors, JAK inhibitors, anti-integrins, or costimulation modulators.
Important Drug Interactions by Category
🚫 Live Vaccines (Contraindicated with Most Biologics)
Live attenuated vaccines — MMR, varicella, live flu nasal spray, yellow fever, oral typhoid — are contraindicated during biologic therapy because the weakened live virus can cause infection in immunocompromised patients. See our vaccines guide for full details.
⚠️ Other Immunosuppressants (Use with Caution)
Combining biologics with high-dose corticosteroids or other systemic immunosuppressants increases infection risk. Short-term, low-dose prednisone is sometimes used during flares under physician supervision. Methotrexate at low doses is commonly combined with biologics and generally considered safe with appropriate monitoring.
✅ Methotrexate (Usually Compatible)
Low-dose methotrexate (7.5–25 mg/week) is frequently combined with TNF inhibitors (Humira, Remicade, Enbrel) and some other biologics for RA. MTX reduces immunogenicity and can improve biologic efficacy. See our methotrexate + biologics guide.
⚠️ NSAIDs and Aspirin
NSAIDs (ibuprofen, naproxen) are generally safe to use alongside biologics for pain relief but may affect kidney function with long-term use. Always inform your prescriber about regular NSAID use. Aspirin at cardioprotective doses is generally compatible with biologics.
✅ Most Common Medications (Generally Safe)
Most common medications for blood pressure, cholesterol, diabetes, and depression have no clinically significant interactions with biologic medications. However, always disclose all medications — prescription, OTC, supplements, and herbal products — to BiologicHealthPlus and your prescriber.
Interaction Considerations by Drug Class
| Biologic Class | Key Interactions to Know |
|---|---|
| TNF Inhibitors Humira, Enbrel, Remicade | No other biologics; no live vaccines; anakinra contraindicated with TNF inhibitors; abatacept combination not recommended |
| JAK Inhibitors Rinvoq, Xeljanz | No other biologics or JAK inhibitors; strong CYP3A4 inhibitors may increase JAK inhibitor levels (e.g., ketoconazole); avoid strong immunosuppressants |
| IL-6 Inhibitors Actemra, Kevzara | IL-6 inhibition can affect CYP450 enzyme activity — warfarin doses may need adjustment; monitor anticoagulants closely |
| IL-23 / IL-17 Inhibitors Skyrizi, Cosentyx, Tremfya | Fewer significant drug interactions than TNF inhibitors or JAK inhibitors; no other biologics; no live vaccines |
| IL-4/IL-13 (Dupixent) | Avoid live vaccines; topical corticosteroids are commonly used alongside Dupixent; no significant systemic drug interactions identified |
