How Does Darbepoetin Alfa Work?
Darbepoetin alfa is a hyperglycosylated analogue of erythropoietin with a longer serum half-life than epoetin alfa. It stimulates erythropoiesis by binding to erythropoietin receptors on erythroid progenitor cells in bone marrow, increasing red blood cell production and reducing the need for red blood cell transfusions.
Dosage & Administration
Chemotherapy-induced anemia: 2.25 mcg/kg SC weekly or 500 mcg SC every 3 weeks. CKD: dosing individualized based on hemoglobin levels and response. Given SC or IV (IV only for dialysis patients). Dose adjustments required based on hemoglobin response.

General Warning
BOXED WARNING: In controlled trials, ESAs shortened overall survival and/or increased tumor progression risk in patients with breast, non-small cell lung, head and neck, lymphoid, and cervical cancers. Use the lowest dose to avoid transfusion. ESAs are not approved for cancer cure or to substitute for transfusion in patients needing immediate treatment. Thromboembolic events. Hypertension. Pure red cell aplasia.
