What is Velcade? Velcade (bortezomib) is the first proteasome inhibitor approved for cancer therapy, used in multiple myeloma (newly diagnosed and relapsed/refractory) and relapsed/refractory mantle cell lymphoma. SC injection has replaced IV administration for most patients due to less neuropathy.

How Does Velcade Work?
Bortezomib reversibly inhibits the 26S proteasome, the cellular complex responsible for degrading ubiquitin-tagged proteins. This blocks the breakdown of pro-apoptotic factors and disrupts multiple signaling pathways (NF-κB, unfolded protein response) that myeloma and MCL cells depend on for survival, triggering apoptosis.
Dosage & Administration
Standard: 1.3 mg/m² SC or IV. SC is preferred — same efficacy, less neuropathy. Various schedules (weekly, twice-weekly) depending on regimen and indication. Part of many backbone regimens (VRd, VCD, VMP, VTd, D-Vd).

General Warning
Peripheral neuropathy — cumulative, can be severe and irreversible (SC has significantly less); dose reduce or discontinue. Herpes zoster reactivation — antiviral prophylaxis recommended. Hypotension. Cardiac toxicity. Pulmonary toxicity. Hepatic toxicity. Thrombocytopenia. Embryo-fetal toxicity.
